Insurance Information

We are providers for many insurance companies. Insurance companies allow you to choose your provider as long as they are approved providers for their company.

We are Tricare Prime providers and accept all Champus and Tricare insurance. Many of our clients are members of ECHO. This site will explain the benefits of this program and the financial aspects.

Tricare requires a prior authorization for the initial evaluation. This is acquired by providing a prescription for the specific therapy from your physician to the Tricare office. Our office can fax these to the Tricare office or they can be placed in the computer with a request to be seen at KidZ TherapEZE. You can check your prior authorizations on your home computer by going to the Humana website. You will need to sign up which takes a couple of days for security clearance.

Children under 21 are covered under THSteps for physical, occupational and speech therapy if they are medicaid recipients. Medicaid does not require prior authorization for the evaluation but does for the follow up visits. To begin therapy you will need a prescription for the specific therapy and your medicaid card. This will link to the Texas Dept of Health web site which has many other programs for children with special needs.

CHIPs covers physical, occupational and speech therapy and requires prior authorization for treatment beyond the evaluation. A prescription is required from a physician and a copy of the CHIPs card.,

BCBS covers physical, occupational and speech therapy but only in a rehabilitative capacity. They do not cover these services if they are habilitative. Each employer plan has different coverage limits and must be clarified on an individual basis.
